Patients living with serious lung conditions are being offered tailored self-management plans by Nottingham CityCare, the city’s largest provider of NHS community services.
With support from the Royal College of Physicians’ National Respiratory Audit Programme, the Integrated Respiratory and Home Oxygen Service (IROS) is holding respiratory workshops for new patients accepted into the service.
Sessions, which are held at the same venues used for Pulmonary Rehabilitation, outline what the service has to offer and the support available to help patients manage their conditions.
After being offered information and the chance to ask questions about nutrition, vaccination, self-management, inhaler technique, stopping smoking and pulmonary rehabilitation, patients can then work with the IROS team to develop a personalised self-management programme.
Clinical Services Manager Victoria Chow said: “We are committed to putting patients at the heart of decision-making.
“By providing them with a full range of information, they can work with our clinicians to develop a self-management plan that more closely meets their needs and personal situations.”
Patients are encouraged to engage in Pulmonary Rehabilitation and are given a nurse clinic appointment for their assessment, if they wish to attend.
Patients who do not attend for personal or medical reasons will receive support to manage their conditions and are able to access other aspects of the service for acute or chronic care.
The work is part of a QI (quality improvement) project being run by the IROS team.
The team is monitoring patient satisfaction and attendance rates at nurse clinics and the transfer of patients from these sessions to Pulmonary Rehabilitation as part of the QI work.

The workshops and pulmonary rehabilitation sessions are held at Bulwell Riverside, St Ann’s Valley Centre, Clifton Cornerstone, The Vine community Centre and The Vale Community Centre, Wollaton.
