Our Diabetes Service is for adults (aged 18 and over) with Type 1 and complex Type 2 Diabetes with joint partnership working with Nottingham University Hospital (NUH). We have a core team of Consultants, Diabetes Specialist Nurses, Diabetes Specialist Dieticians, Clinical Support Workers, Diabetes Educators and Administrators.
We aim to provide all-round approach of care for people with Diabetes to help them self-manage their long term condition
We provide telephone support Monday to Friday between the hours of 9:00-16:00 for advice for healthcare professionals and patients who are registered with the service.
We deliver clinics from GP practices and Health centres across the city to help ease of access to the citizens of Nottingham as well as providing support for CityCare community teams i.e. District Nursing and care homes team.
Our service includes:
- Signposting to other diabetes services
- Triage service
- Multi–disciplinary clinics
- Provide support and education to City GP practices to help them to manage their patients with Diabetes
- Provide structured education (DAFNE/ DESMOND) or one to one sessions for people with diabetes
- continuous blood glucose monitoring
- completion of all nine care processes following year of care approach
- review of EMAS referrals following call out for severe hypoglycaemia
- goal setting
- injectable therapy initiation
- review of complex treatment regimes
- carbohydrate counting
Type 1 Referral Criteria:
- Confirmed diagnosis of diabetes type 1 (newly diagnosed within the first year should be referred to NUH)
- Registered with a Nottingham city GP practice
- Consent to referral received and healthcare records shared
Type 2 Referral Criteria:
- Confirmed diagnosis of diabetes type 2 NOT newly diagnosed
- Patient should be taking maximum tolerated oral therapies, blood glucose testing and still continue to have unstable blood glucose control
- Attended diabetes structured education: DESMOND (for patients with type 2 diabetes NOT on insulin), DESMOND injectable unless housebound.
- Registered with a Nottingham City GP practice
- Consent to referral received and healthcare records shared
